Before Enrollment:
1. Check Child Care Information and/or Universal Pre-Kindergarten page for required documents needed for enrollment.
2. Gather required documents together. If you need a salary verification form or a vocational training form from the center, please stop by the center before enrollment appointment to pick one up to have it completed by your employer or school.
3. To save time at appointment, copy all children's birth certificates, and social security cards. If you receive pay-stubs, please place your paystubs in order and put them in an envelope. If you cannot find all of your paystubs, try to obtain them from your employer's payroll office. If that is unsuccessful, pick up a salary verification form, have it completed by your employer and bring the completed form with the paystubs you have.
4. If you receive court ordered child support, make a copy of the child support order or the printout of child support payments (available online) to bring with you to the appointment.
During Enrollment Appointment
For Parents on Public Assistance: Bring form W274 to Ms. Forde, Director and ask her to complete it. Pick up a medical form and schedule a medical appointment for your child immediately. After W274 form is completed, return form to your caseworker.
For parents not on public assistance:
1. Submit required documents to the director. Any original documents submitted will be copied and returned to parent. You will be asked various questions to assist with completion of the enrollment paperwork. Be prepared to spend at least 10 minutes with the director.
2. Pick up a medical form, which must be completed by your child's doctor. To save time and to get an earlier start date once you receive a placement notice from ACD, schedule a medical appointment immediately.
3. Before you leave the center, you should make sure you have all of your documents and belongings with you.
After Enrollment Appointment
For parents on public assistance: Return form to your caseworker. Ask caseworker whether you will receive placement notice in person or via mail. Once you receive the placement notice, hold onto it until your child's medical form is completed. When you have both documents, bring them to our center between 8am and 5:30pm in order to get a start date for your child.
For parents not on public assistance:
1. After the director completes the paperwork and attaches it to the required documents, she will bring them to the ACD Resource Area Center. ACD (Administration for Child Development) will process the paperwork and send you a placement notice in the mail. Your child's medical form should be completed as soon as possible since the enrollment paperwork will take about 2-3 weeks to process. If the medical form is not complete when you receive the placement notice, contact the center at 718-991-1050 to inform Ms. Forde that you are awaiting the completion of the medical form. Once the medical form is complete, bring the medical form and the placement notice to our center in order to get a start date for your child.
We will not be able to accept a child without the medical form!
Dates and Times of Enrollment: While we prefer that parents call ahead of time to schedule an appointment, parents can walk in between 10am and 5:30pm from Monday through Friday. Please call 718-991-1050 to make sure that Ms. Forde, Director is available for enrollment.
